CynoCentric, LLC will endeavor to create as safe an environment as possible for the training of my dog and will endeavor to offer only sound, safe, and responsible training and training instructions. However, I recognize that CynoCentric, LLC is not responsible for any unintentional errors, omissions, or incorrect assertions. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

Further, I am and will remain responsible for the actions of myself and my dog at all times. I hereby agree to abide by the rules and policies of CynoCentric, LLC as set forth in this contract.

I understand that my dog must remain on leash, or tethered, or crated unless otherwise instructed by the Lead Trainer.  If I do not think I can safely restrain my dog, I will ask for help immediately.

I will not allow my dog to greet another dog unless otherwise instructed by the Lead Trainer and only with the explicit permission of the other dog’s owner.

I will present proof that my dog is current on vaccinations for rabies, distemper, adenovirus, and parvovirus, or I will present a statement from my licensed veterinarian recommending a specific exception.  I will keep my dog under the care of a licensed veterinarian.  My dog has been seen by a veterinarian within the last 12 months.

I will not bring my dog to class if she/he has been vomiting, coughing, has diarrhea, is listless, lethargic, is not eating, or if she/he has yellow or greenish discharge from her/his eyes or nose.  I will see a veterinarian for these conditions and will not bring my dog back to class until the veterinarian has treated and resolved the issue.

I will not hand my leash to anyone, including a Trainer, unless I am absolutely comfortable doing so.  If my Trainer asks to use my dog to demonstrate a technique in class and I do not want them to handle my dog, I will say so.  I understand that I do not need to provide a reason to maintain control of my own dog.

I understand that some dogs are on special diets, have allergies, or have different rules about food.  I will not feed any dog other than my own unless specifically instructed by the Lead Trainer, or authorized to do so by that dog’s guardian.

I will bring my dog to class and private sessions on properly fitted and secure equipment.  I understand that CynoCentric, LLC recommends that I bring my dog on a front-attaching harness, flat-buckle collar, head-halter, regular harness, or martingale collar.  If I do not think I can safely maintain control of my dog, I will discuss this with my Trainer before coming to class or private session.

I understand that cancellations made 48 hours or more in advance of the start of the group class series or private lesson will receive a refund less a 25% cancellation fee.  I understand that cancellations made later than this are not normally eligible for a refund.

I understand that attendance of dog training classes, lessons, seminars, trials, etc. is not without risk to myself, members of my family, guests who may attend, or to my dog. In consideration of, and as inducement to the acceptance of my application for training membership in this dog training class, lesson, seminar, trial or other service, I hereby agree to indemnify and hold harmless CynoCentric, LLC, its officers, partners, directors, instructors, trainers, agents, employees, volunteers and/or representatives of any and all claims, or claims by any member of my family, or accompanying guests of mine of injury, expense, costs or damages to myself, my dog or any handler sponsored by me both in class/seminar/lesson and out of class/seminar/lesson. In addition, I agree that I will defend and indemnify CynoCentric, LLC and its officers, partners, directors, instructors, trainers, agents, employees, volunteers and/or representatives for any injury, expense, costs or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my own actions or the actions of my dog. I have read the above-stated provisions, and agree to accept those responsibilities.
